If you have any difficulty opening the form, please contact our office at one of the phone numbers below.  After we receive the form in the office, we will contact you to schedule a time. The information boxes to the right side of the page are for contact or request information only.

Phone appointments are done Tuesday through Thursday from 7:00am to 11:00am PST
Private in person sessions are done during the time of the workshops in the city he is visiting. There are a limited number of private sessions available in Los Angeles. Please contact our office for details or you can use the contact fields to the right with your name, phone, or email address and the best time you can be reached.
If contacting by phone, please let us know the best time to reach you. Dr. Yuen's appoinments are booked at least 3 weeks in advance. 

We accept Visa, MC and Discover. Sorry but we do not take AMX. If you would like to send a money order please contact our office for prices. Money orders for appointments should be made out to CEM.
